1. NAME OF PROGRAM: Personal Care Services (PCS)

2. PURPOSE OF PROGRAM: This program covers aide services in private residences. The aides perform personal care tasks for patients who, due to a medical condition, need help with such activities as bathing, toileting, moving about and keeping track of vital signs; and housekeeping and home management tasks that are essential, although secondary to the personal care tasks necessary for maintaining the patient’s health.

3. APPLICATION PROCEDURES: Application for PCS services is made with the Cabarrus County Department of Social Services through the Adult Service Intake unit. Applications may be made by phone for individuals who currently have adult Medicaid coverage. Anyone who does not already have adult Medicaid coverage will need to apply in person to complete a Medicaid application and make request for CAP/DA services. If unable to come to the agency, a representative may apply on the applicant’s behalf.

4. ELIGIBILITY REQUIREMENTS: The eligibility requirements include:

1. The individual must be covered under regular Medicaid coverage (blue card) or Pregnant Women coverage (pink card) and require PCS due to a pregnancy related condition.
2. The individual must have a medical condition that requires the direct and ongoing care of the physician prescribing PCS.
3. The individual must be medically stable and at the maintenance level.
4. The individual must need help with personal care tasks due to the medical condition.

5. OTHER PROGRAM DETAILS: The other program details include:

1. PCS must be the most cost-effective and appropriate form of care. PCS is to assist, not replace, the help available from family members and community resources.
2. The amount of PCS covered for a patient in a calendar month is the amount of PCS aide services provided according to the plan of care authorized by the physician plus any billable RN time, up to a maximum of 80 hours total time for the month.
