1. NAME OF PROGRAM: Foster Care Placement Services for Adults

2. PURPOSE OF PROGRAM: The purpose of this program is to assist adults (age 18 or older) or their families in securing placement for the adult into a long term care facility. This includes placement into an Adult Care Facility/ Assisted Living Facility (rest home) or placement into a nursing home (skilled or intermediate care).

3. APPLICATION PROCEDURES: Application for assistance with placement services is made with the Cabarrus County Department of Social Services through the Adult Service Intake unit. Applications may be made by phone or in person. If financial assistance is needed for facility care cost, the individual or responsible party will need to apply in person to complete a Medicaid application (nursing home) or Special Assistance application (rest home).

4. ELIGIBILITY REQUIREMENTS: The eligibility requirements include:

1. The individual must be in need of placement into a long term care facility. A physician must complete the required medical form (FL-2) and check the level of care needed (domiciliary care for rest home and ICF or SNF for nursing home).
2. Assistance will be provided in identifying the homes providing the level of care recommended by the physician.
3. A social worker will be available to work with the individual and/or family members to work out the details of the placement arrangements.
4. This service is provided without regard to income. Eligibility is based solely on the documented need for the service.

5. OTHER PROGRAM DETAILS: Staff in the agency is also responsible for monitoring homes located in the county licensed as Homes for the Aged, Family Care Homes or Group Homes for the Developmentally Disabled. As part of the monitoring, investigations of complaints regarding these homes are conducted. Anonymous complaints are allowed by the agency.
