1. NAME OF PROGRAM: Special Assistance Demonstration Project (SA Demo)
2. PURPOSE OF PROGRAM: This program grew out of a desire to offer home care as an alternative to persons 18 and older to remain in their own home rather than placement into a adult care home (rest home). SA Demo Project can provide financial assistance to adults facing the prospect of moving from their own home due to difficulty meeting all needs to assure health, safety and well-being. The funds can be used to meets many needs including: excess utility costs, excess medication costs, home mobility aids which include wheelchair ramps, non-skid surfaces and widening of doorways; telephone emergency alert, medical supplies, pillboxes, home delivered meals, and nutritional supplements taken by mouth such as Ensure, Boost, etc.
3. APPLICATION PROCEDURES: Application for SA Demo Project services is made with the Cabarrus County Department of Social Services through the Adult Service Intake unit. Anyone interested in this program will need to apply in person to complete a Special Assistance for Disabled Adults (SA) and make request for SA Demo Project services. If unable to come to the agency, a representative may apply on the applicant’s behalf.
4. ELIGIBILITY REQUIREMENTS: The eligibility requirements include:
- The individual must need the level of care provided by an adult care home (rest home) and desire to remain in their own home. A physician completes the required medical form (FL-2) to document the need for adult home level of care.
- The individual must have income less than or equal to the current poverty level and be eligible for full Medicaid benefits (blue card).
- The individual must have his health, safety and well-being assured while remaining in his home.
- The amount of benefits received will be based on need as determined by an assessment and service plan completed by a social worker.
5. OTHER PROGRAM DETAILS: SA Demo Project exists to supplement rather than replace the formal and informal services and support already available to an individual. The program’s goal is to enable individuals to remain in their own homes or to return to their homes from adult care homes and live as independently as possible. SA Demo Project serves persons living in the Cabarrus County area only.
