1. NAME OF PROGRAM: At-Risk Case Management Services
2. PURPOSE OF PROGRAM: The purpose of At-Risk Case Management Services is to help families to remain stable and self-sufficient. The social worker provides or arranges a broad range of counseling and other services to support families with children. The social worker has responsibility for locating, coordinating, and monitoring appropriate community services for the client. The family situation is assessed and a written case plan is developed. A variety of medical, education, and human service agencies in the community are contacted so a comprehensive service plan may be completed. The worker coordinates and monitors services to determine that the client received the service, that services are of good quality and that services meet the needs of the client.
3. APPLICATION PROCEDURES: Applications are taken in the Crisis Unit at the Department of Social Services. Applications may also be taken through home visits and at certain outreach locations.
4. ELIGIBILITY REQUIREMENTS: Applicants must be receiving Medicaid and have a need for the service. They must be residents of Cabarrus County.
5. OTHER PROGRAM DETAILS: At-Risk Case Management Services are funded entirely through the Medicaid program.
