1. NAME OF PROGRAM: Transportation Services
2. PURPOSE OF PROGRAM: The purpose of Transportation Services is to provide access for individuals to medical providers, job seeking, education, training and other activities that are required for daily living, when access is not otherwise available.
3. APPLICATION PROCEDURES: Anyone requesting Transportation Services should either come to the department of social services or call to request the service.
4. ELIGIBILITY REQUIREMENTS: The various transportation programs at DSS have different eligibility guidelines as follows:
- Medicaid Transportation: This is authorized in certain Medicaid programs for recipients who are authorized for Medicaid and require this service to access medical providers. It is not authorized if it can be provided by another means. In some instances, where cost to a recipient is a barrier to service, they can be reimbursed for the service.
- Social Services Block Grant Transportation (SSBG): This service is based on income and need and can be used to assist an individual in accessing transportation when it is needed to help them access resources that are needed in their regular activities of daily living.
- Elderly, Disabled and Handicapped Transportation Program (EDHTAP): This is based on need and income, and can be used when the individual requiring transportation for medical needs is not Medicaid eligible.
- Work First Transportation: This is transportation based on either eligibility for the Work First Employment Program or families meeting the guidelines of 200% of the federal poverty level. The purpose of this is to assist families in accessing jobs or training that leads to self-sufficiency.
5. OTHER PROGRAM DETAILS: SSBG, Work First and EDHTAP are based on need and eligibility and can be used for an indefinite period of time. Medicaid Transportation requires an annual review to re-determine eligibility and need for the service.
